Asking someone's name and introducting ourselves is something we all teach at the beginning of a Chinese course. Here is a page from the book 'Rhythm and Tones' by Pauline Huang and Robin Harvey. This name chant is something that I get my students to listen to once or twice and then I do it with the whole class. It is an effective way to teach the phrase "ni jiao shenme mingzi?" You will have to get hold of a copy of the book to listen to the tune.
